The role of advanced registered nurses has undergone significant transformations over the years, with shifts in scope and expectations occurring throughout the 20th and 21st centuries. As per the Consensus Model for APRN Regulation, advanced practice registered nurse (APRN) roles currently encompass nurse practitioners, certified nurse-midwives, certified registered nurse anesthetists, and clinical nurse specialists. This evolution of roles began over 150 years ago during the American Civil War when Nurse Anesthetists administered chloroform to wounded soldiers. This role continued through various wars and later expanded into rural healthcare settings.

In 2021, there were approximately 43,950 certified registered nurse anesthetists providing anesthesia care across the United States. The evolution continued with the establishment of the first midwifery school in 1925, focusing on enhancing maternal and infant outcomes. By 2019, there were 11,826 certified nurse-midwives offering comprehensive care to women during pregnancy, childbirth, the postpartum period, family planning, and gynecologic needs.

The 1960s saw the development of the nurse practitioner role, primarily in response to the shortage of primary care physicians. As of 2022, there were approximately 355,000 licensed nurse practitioners in the United States, serving in various specialties such as adult-gerontology, family, acute care, pediatric, women’s health, and psychiatric care. Additionally, the clinical nurse specialist (CNS) role emerged in the 1960s to address the growing need for psychiatric patient care. However, in recent years, many hospitals moved away from supporting this role due to cost considerations and challenges related to billing for CNS services.

The American Association of Colleges of Nursing (AACN) recognizes that APRN roles extend beyond direct patient care to include management of care for individuals and populations, administration of nursing and healthcare organizations, and the development and implementation of health policy. This recognition encompasses roles such as nurse administrators, public health nurses, nurse policy makers, nurse educators, and clinical nurse leaders. While some of these positions require a baccalaureate or master’s degree, there is a growing push for nurses to pursue a Doctorate in Nursing Practice (DNP) to be considered APRNs.

Looking ahead, APRN roles are expected to continue evolving to meet the healthcare system’s changing landscape. The growing focus on cost containment, the shortage of primary care physicians, and the demand for high-quality care will drive this evolution. For instance, the Bureau of Labor Statistics projected that by 2029, the Nurse Practitioner (NP) role would experience a 52% growth compared to a 10% growth for physicians. In 2011, the Institute of Medicine (IOM) released a report emphasizing the need to place nurses at the forefront of healthcare. It is anticipated that new APRN roles will emerge to address the demand for coordinated care, the increasing complexity of disease processes, the aging population, and the necessity for health promotion and disease prevention.
