This course examines nursing theory and the role of ethics for advanced registered nurses within the Christian worldview and through a leadership perspective focused on improving health care outcomes. Students explore the moral/ethical responsibilities and legal and regulatory obligations of advanced registered nurses in health promotion and disease prevention. Students also review evidence-based practice (EBP) literature and the research process with application to their program of study and learn to navigate scholarly EBP literature , resources, and guidelines.

The role of advanced registered nurses has undergone significant transformations over the years, with shifts in scope and expectations occurring throughout the 20th and 21st centuries. As per the Consensus Model for APRN Regulation, advanced practice registered nurse (APRN) roles currently encompass nurse practitioners, certified nurse-midwives, certified registered nurse anesthetists, and clinical nurse specialists. This evolution of roles began over 150 years ago during the American Civil War when Nurse Anesthetists administered chloroform to wounded soldiers. This role continued through various wars and later expanded into rural healthcare settings.

In 2021, there were approximately 43,950 certified registered nurse anesthetists providing anesthesia care across the United States. The evolution continued with the establishment of the first midwifery school in 1925, focusing on enhancing maternal and infant outcomes. By 2019, there were 11,826 certified nurse-midwives offering comprehensive care to women during pregnancy, childbirth, the postpartum period, family planning, and gynecologic needs.

The 1960s saw the development of the nurse practitioner role, primarily in response to the shortage of primary care physicians. As of 2022, there were approximately 355,000 licensed nurse practitioners in the United States, serving in various specialties such as adult-gerontology, family, acute care, pediatric, women’s health, and psychiatric care. Additionally, the clinical nurse specialist (CNS) role emerged in the 1960s to address the growing need for psychiatric patient care. However, in recent years, many hospitals moved away from supporting this role due to cost considerations and challenges related to billing for CNS services.

The American Association of Colleges of Nursing (AACN) recognizes that APRN roles extend beyond direct patient care to include management of care for individuals and populations, administration of nursing and healthcare organizations, and the development and implementation of health policy. This recognition encompasses roles such as nurse administrators, public health nurses, nurse policy makers, nurse educators, and clinical nurse leaders. While some of these positions require a baccalaureate or master’s degree, there is a growing push for nurses to pursue a Doctorate in Nursing Practice (DNP) to be considered APRNs.

Looking ahead, APRN roles are expected to continue evolving to meet the healthcare system’s changing landscape. The growing focus on cost containment, the shortage of primary care physicians, and the demand for high-quality care will drive this evolution. For instance, the Bureau of Labor Statistics projected that by 2029, the Nurse Practitioner (NP) role would experience a 52% growth compared to a 10% growth for physicians. In 2011, the Institute of Medicine (IOM) released a report emphasizing the need to place nurses at the forefront of healthcare. It is anticipated that new APRN roles will emerge to address the demand for coordinated care, the increasing complexity of disease processes, the aging population, and the necessity for health promotion and disease prevention.

In the 20th century, advanced registered nurses operated in an environment where health technology played a limited role. However, the 21st century has witnessed a paradigm shift, with advanced healthcare technologies becoming central to the delivery of high-quality, safe, and efficient care to diverse patient populations.

During the 20th century, advanced registered nurses primarily provided fee-for-service care. The focus of nursing care was often on the volume of care delivered to patients, which sometimes resulted in significant costs for patients seeking healthcare services. In stark contrast, the 21st century has ushered in a new era of healthcare, where advanced registered nurses prioritize value-based care. This transformation emphasizes the achievement of quality and patient-centered outcomes (Gullatte, 2018). The key outcomes of this shift include patient-centered care, cost-efficiency, and enhanced safety in healthcare delivery.

Looking forward, the roles and responsibilities of advanced registered nurses are poised to continue evolving and emerging in response to the changing landscape of the American healthcare system. Advanced registered nurses will increasingly harness the power of big data to make informed decisions that address the current and future needs of their patient populations. Anticipated shifts in the demographics of the American patient population will require nurses to be highly adaptive and responsive to the evolving care requirements of diverse patient groups. Moreover, technological advancements and evolving legal frameworks in the United States will play a crucial role in shaping the future roles and responsibilities of advanced registered nurses (Young et al., 2017).

To meet these changing needs, advanced registered nurses will need to ensure that the care they provide is evidence-based, innovative, efficient, and patient-centered. This approach will be pivotal in addressing the diverse and dynamic healthcare needs of the evolving patient population.
